Have you used Recover My Files from www.recovermyfiles.com?

Default Data Recovery

When you run a Complete Format Recover, Recover My Files will find full file and folder names and display this information in "Folder View" of the results screen.

It is possible that the file and folder name information has been destroyed and can no longer be recovered.
